1. Graduate of an accredited physical therapy assistant program with either an Associate’sdegree in science or applied science that is approved by American Physical TherapyAssociation, Inc.2. Possesses and maintains current CPR certification.

- Possesses a degree from a baccalaureate or masters program in physical therapy approved by an accredited organization.- Licensed to practice as physical therapist by the Physical Therapy Board of California.- Two years of appropriate experience as a physical therapist. Is licensed as a speech language pathologist by the State in which the individualfurnishes such service; orB. If the State does not license speech-language pathologists:1. Must successfully completed 350 clock hours of supervised clinical practicum (oris in the process of accumulating such supervised clinical experience);2. Performed not less than 9 months of supervised full-time SLP services afterobtaining a master’s or doctoral degree in SLP or a related field; and3. Successfully completed an approved national examination in SLP2. A minimum of one year's clinical experience in Speech–Language Pathology.3.
